Dental implant, who should do it?

I had a root canal and Apico now am looking forward (ha) to a dental implant. The original Apico was done by an endodontist, and he did originally did tell me it might fail, after getting infected and being on antibiotics, but has been fine for several years. Recently my regular periodontist told me that the tooth did not look good, and said I might need a bridge or implant, neither of which he does. So he sent me to my dentist and he discussed it with me, and since a bridge is not a good idea -it is a close to front tooth and the bridge would mean ruining 2 good teeth-- he advised me on the implant. The dentist does not do it either but recommended me to his periodontist, who is at his office once a week. My original periodontist, was recommended by him several years ago, guess before he had one on staff. I asked about going back to the original endodontist, and he said that is up to me that they (this perio, and the endo) both could do it , it is up to me. Which do I choose?

I would go to whoever was most qualified to do the job correctly the first time around. You may wabt to expand your search and look for others who specialize in this type of dentistry. Just research it very well as not all dentists who do this sort of thing are good at it.
Reply:I have never heard of endodontists doing implants. But I definitely have heard of periodontists doing them. Periodontists are usually very careful with soft tissues (gums), so I would recommend the periodontist, especially if it is in an esthetic area.

Anyone with a MISSING "I" tooth?

I'm 16 and I have one missing "I" tooth. A couple of years ago my orthodontist said all he would have to do is reshape it, but he had to wait a couple years to let my teeth develop. He also said that I would not need braces. So, I went back months ago, and he found that I have a cross bite, so that would require me getting braces on my bottom teeth, and also that I would need to have an implant for my missing "I" tooth.


So, what I don't understand is why is it that before he


I needed to reshape that area, but now I need an implant?


Also, is an implant the best way to go? HAS ANYONE been in a similar situation!!!!

It is hard to give you a good answer without all the information (actually seeing your teeth, models, x-rays, etc.). Generally speaking, I would think that either choice would be a reasonable treatment plan for someone missing one canine ("I" tooth). Like I said, without the specific information it's hard to say which way I would recommend for you. I think you might want to go get a second opinion and talk to them about the two ways your orthodontist has told you about. Often, when getting a second opinion the dentist will be more free to explain the options because they won't be the one performing the treatment--so be sure and tell them that you'll be having whichever treatment you choose done by the other orthodontist, otherwise they might spend all their time trying to convince you to come to him or her instead. You should tell the second opinion orthodontist exactly what your orthodontist said and have them explain the good and bad things about each option.

I am missing the tooth next to my front tooth. never had it. Grew up feeling fine until I went to a dentist and they made me start this whole braces and implant thing.





I have been in this whole braces fiasco for 4-5 years. I was happier without my tooth. Now I have a huge gap, that they opened up for the implant, and I am forced to live with that until I am done with treatment.





SUCKS. If your okay with it... then let it be.

I don't have insurance so i said "what the hell, let's give Mexico a try", I went there to get my 4 wisdom teeth removed and I paid about half what the greedy dentists wanted here in the States, everything went fine.





I killed two birds with one stone, I had a vacation and the wisdom teeth removed!!





Anyways, having an implant sounds complicated, so I would search online for dentists in Mexico, and e-mail them or call them and ask them for more info, like how long you would need to stay in Mexico.

If you have health insurance, talk with them about adding a dental policy to your plan. It will usually takes up to six months to take effect on dental work, but automatically kicks in for cleanings.
Reply:The dental insurance plans I have looked at do not cover implants, if you find one please let me know. If I were you I would approach the dental school I know that some school offer free care for work by students who are under supervision of a dentist. You could aslo consider going south of the border since you are in California as you will still have to pay a load even with dental insurance.

Implant options and would you do it?

My crown recently broke and the tooth underneath was all decayed so there was nothing that could be done but to take it out. So I was wondering what my options are and if you think I should get an implant. The place where the tooth was is not visible when I smile because its in the back. I had an issue with my wisdom teeth when I had the lower ones out the top ones started growing out and hitting my gums I am concerned about this. Whats your input? thanks!

Do it...you look like you are young and have plenty of life left in your teeth! The problem with a bridge is that you have to basically destroy the adjacent teeth to the one being fixed to attach the bridge. If you leave the space empty all the other teeth on the opposite arch as well as the teeth next to it will shift around to move in the empty space. This will mess up your bite! So, if you want the best thing, do the implant.
Reply:Implants are great (I have one), provided your body doesn't reject it. Patients have to qualify, or be a good candidate, for an implant. Bone structure and density are what the Docs look for to approve the treatment. To find out if you are a candidate, a panoramic xray will be taken... this is the kind that goes around your head/face area.





The tooth above (below? you didn't specify where the tooth that was extracted was located) the space where the tooth was will start to drift into to area, since there isn't anything to keep it from doing that.





Insurance companies will not cover the cost of an implant, so it's out-of-pocket expenses.. they will cover a portion of the bridge fee.

Tooth extraction again?

What if I leave 6 teeth bottom front and get a partial plate for the back with maybe 2 back implants for stability,then remove all top teeth and have a full denture.I am 50 years old and have looked after my teeth but when I was young a dentist filled all my teeth so fully and because I clench my teeth I have cracked them,I live dailly waiting for pain and I am too scared to eat anything harder than toast,I only eat chicken or soft meat,the reason I want my teeth out is because this is ruling my life at the moment.I would rather not get them out but the pain is like chewing on alfoil.

Seems like there are a lot of people our age who had bad dentistry when we were young. My dentist did the same to my teeth - it STILL annoys me even now and I'm nearly 50! Anyways, have you thought about mini implants? Apparently, they're a lot, lot cheaper than individual implants and they can be used to anchor a whole set of teeth and also they preserve the bone so you wont get that sunken look. Otherwise, you could get a couple of implants at the back on each side and then get a bridge fitted. I'd try and leave your front teeth there if possible. That's just me though - probably best to talk to a good dentist. Isn't it great these days that even if you have to have your teeth out, you can now get implants done!!
Reply:I feel so bad for you.. Ask your dentist what would be best for you. Sorry but anything "we" tell you on Yahoo Anwers probably wont be the best advice as we are not dentist and even if we were we would need to see you in person. Explain your story to your dentist and he'll know best.

Reply:Only have them pulled out if they cannot be fixed. Remember, once they're gone, that's it. you can't get them back or change your mind.





Taking the bottom ones (side ones) and replacing with half denture is a good idea. But I find people who wear top full denture generally distort their looks. If it is possible, leave the top front ones and have the side ones, the teeth you chew on, removed. It is possible to have partial denture, leaving the front one for aesthetic reasons.





You shouldn't live in pain. Obviously you won't enjoy eating your food, a basic human necessity.





Discuss the above with your dentist but remember, tooth implant is very very expensive and you don't need them to anker a partial denture.





It is a very big step but don't let it ruin your life. Go see a good dentist and am sure there are sometimes alternative to pulling the teeth.

Implant or Bridge?

I broke my front tooth and the dentist suggested an implant or a bridge to replace it. My other front tooth is fine, and the tooth on the other side of the broken one already has a root-canal so I don't know which to choose. The bridge will be cheaper, but I'll have to sacrifice my good front tooth to replace the missing one.





My dentist recommended the implant because she said after the tooth is removed the bone will start to decay and the implant will prevent this. Has anyone else had a bridge on their front teeth? Did your gums start to sag over time or anything?

i suggest going for the implant like your dentist recommended. You don't want to damage a perfectly fine tooth, the one next to the broke one. and the one on the other side being alredy root-canal treated is a "dead" tooth and they can be quite brittle. so you have to remember it might risky.





The bone doesn't start to decay but it shrinks and may cause you're other teeth to move slightly.

I think you should tell her a little bit about your past, don't show any pictures. Just say what you've just said to us, and see her reaction... if she does not care then you can now calm down and know you guys were meant to be. However if she seems like she's going to reject you (don't decide for her, let her say it-if she says i'm not interested thn okay... don't put words in her mouth- eg: she probably rejected me because she hasn't talked to me yet) thn that's ok, you will probably find someone out there.





Don't lie about anything because she won't fall in love with YOU she'll fall in love with the FAKE YOU. I think you should tell her now before you start liking her even more and becoming attached.

Reply:hey. what you went through is really horrible. this is a pretty hard question to answer. usually i would say that if she really likes you, she won't care what you look like but nowadays, I'm not so sure. you could slowly tell her some stuff that's happened, just so you can get her informed. don't go stuffing it all into one e-mail, but tell her bit by bit, slowly and carefully. you can even ask her "would you care how i look if you ever saw a picture of me?" and see what her response is. if you two do meet than i guess you'll find out what she thinks then. but even if she doesn't feel the same way about you as you do her, remember that their is someone for everyone, you included.
Reply:hello. i understand how difficult this must be for you. spilling your most intimate fears online for all to see. i don't know if what i have to say will help or not, but i mean it with the utmost care and support. from the sounds of things, you have some major issues with your past and health that need to be delved into before you can be healthy enough to be healthy for this cute, chubby readhead, friend of yours. if she really cares for you, then she cares; but you need to sort out your own issues first so you can be healthy to have a successful relationship with her. it sounds like you have difficulty making emotional connections that will last with others and that is a very big component of relationships, all of them, regardless of gender. ptsd is serious. if you are suffering from it, you need to get help. perhaps from a counselor or clergy, since you don't have other relationships to fall back on. something that detailed and involved should be dealt with by a professional that can help you work through things that are currently obstacles, and put them in a place where you can continue on your road to happiness. roadblocks that big can not remain or the car will break down trying to pass them and then you will be stuck, on the road, in the middle of nowhere, with no way to get where you are going. pardon the metaphor, but it's true. i saw something when i was very little, and it has stuck with me; and i use it in my own life's difficulties, "life is hard, yard by yard. inch by inch, it's a cinch". i don't know who the author is, but it is true. one step at a time.

Has anyone had a Maryland Bridge?

Im looking for real life experience on the functional ability as well as the aesthetic value. I am missing four teeth and have been considering implants but that also includes bone grafting for all four sites and I really just cannot afford it I am afraid. I have decided already to havea Maryland on the bottom but Im thinking now, "why not on the top as well?" Any help would be wonderful. Thanks :)

Has anyone had a Maryland Bridge?
Anyone that would make you a Maryland bridge to replace 4 teeth should be shot! That would be so below the standard of care of dentistry -borders on malpractice- it isn't funny. Maryland bridges consist of a wing attached to adjacent teeth ( on either side) of the missing tooth. To replace 4 teeth on Maryland Bridge would fail within a year, probably less. If finances are a concern and implants are not in your budget, consider a partial denture. You can always have implants down the road. Under no circumstance put a Maryland bridge in, it will not hold. Typically, a Maryland Bridge is meant for short term basis 4-5 years at best and under certain restrictions, and not as good as permanent fix like a bridge, denture or implant. Good luck with your decision


***There is no such thing as a temporary Maryland Bridge***


To replace four separate laterals is:


#1) costly- each maryland bridge will span 3 teeth


#2) hygienically okay, but it means a lot more fastidious cleaning for you, they will act like splints and if plaque and bacteria sit for long period (without being able to clean and floss well) you stand the chance of developing periodontal problems.


Implants really would be your best option- longest and best prognosis without having to touch the adjacent teeth
Reply:A Maryland bridge only has wings on either side of the false teeth that are getting replaced so if you are replacing 4 missing teeth, that would be an immense amount of strain on the wings of either side of the bridge. It is only usually used when one tooth is missing and a palate is not wearable for the patient. Is your gap 4 teeth wide and if so maybe a fixed bridge would be the better solution for you. This is where the dentist shapes down the teeth at either side and the puts a complete bridge over the teeth and the gap. Hope this helps ... but just advice on the Maryland ..don't touch if over 2 teeth gap. It wont hold.
Reply:This type of bridge don't have a good reputation because the retention is weaker than other types, u can consider having a partial denture done as a temporary measure as it is much cheaper than bridge, but implant is the best option to replace missing teeth.

Reply:implants would be your best choice for long term function and it is also cost effective to do them now because more than likey a maryland bridge will need to be replaced multiple times in your life, if you are young. If implants are not an option at all then a maryland bridge is a very acceptable alternative, and it can be extremely esthetic as well. The final results are determined by your dentists ability, the labs ability and your oral health as well as your bone and gum structure. If you are seriously considering a maryland bridge then try a temorary on first, this will cost you more upfront but can save you more expense in the long run. your dentist can bond some denture teeth onto orthodontic wire and then bond this to your natural teeth. This is a good way for you to try this type of procedure out first and if you like it then you can go with a traditional Maryland Bridge. A temporary might not be possible it really depends on your bite but most people can have one. Remember the final one will always look better than the temporary and also remember that there will be a fee for having a temporary one as well. I would talk to you dds about it and see what they think. Best of Luck

Reply:Maryland bridges are cheap but very unreliable. If you want to bite on the teeth they will last less than a year (on average) they are fantastic aesthetically but not very functional.





Implants are expensive and invasive but they are definately the better option of the two

Full mouth dental work?

I ask this question now, as the last time this question seems to have been answered is a year ago, and a lot can happen in a year, price/procedure wise, as I know now that there are a variety of different options available that were not before, such as crowns in an hour, etc, whereas before it took months.





I am therefore asking, if I was looking to have all my teeth extracted, and replaced by implants, what is the cheapest I could be looking at for all that work? I would consider either USA or UK, depending on the price - I am a UK citizen, so yeah I am just trying to find out a price from a cheap place before I call around - I have only found 1 place here, and 2 in USA, and I can't really go to the USA ones easily in person...

Full extraction with implants is a bit extreme. I can't speak about price, since I'm a US dentist. I'm sure this procedure would fall under the UK mandate of destroying the invasive bone drill, rather than sterilize it (since sterilization doesn't inactivate prions). That will increase the cost. Aside from that, the management of your bite (the point at which tthe teeth actually bite through food) is a difficult thing to manage and is one of the common complaints with people with dentures.


You don't say how many teeth you have but you can count on the cost of extractions, bone reconstruction where needed,sinus lift surgery where needed, healing time, placement of implants, temporary denture during healing, abutment placement, fabrication of dentures (if implant retained dentures.. fewer implants needed) or crowns (if individual teeth are made...more implants, more expense)


and finally, adjustments. You're looking at like a year's worth of work or more

Reply:There are so many variables that an answer would be almost meaningless. Obviously your teeth must be in poor shape if you're considering this option. You must also think that implants are fool-proof (which they aren't) or that every patient can have all their teeth replaced with implants (which they can't). You may be able to do this, but perhaps need bone grafting, sinus lifts and other imponderables that will make the course of treatment more involved and expensive.


With these caveats I would say that for standard (not mini) implants about the lowest fees you're likely to find in the U.S. per tooth would be maybe $1500-$2K for the implant fixture, and maybe $1000 to $1500 per tooth for abutment and crown. So you're looking for $2500-$3500/tooth minimum (not counting the extractions)

As they do a lot of cases (high volume), they are able to offer the job at a shorter lead time too. The lower cost does not mean lower quality. It is mainly because of exchange rate and cheaper labour cost.





The cost for tooth extraction is only about $20-$30USD per tooth.





The cost for implant and crown is only approx $2000 USD per tooth.





All this can be done in 1 trip to either Bangkok or Phuket and you only need to stay there for 10-14 days.

I'm frustrated, finding a girl?

I dont have any experiance with girls. I want to date but I have a lot of problems that I feel need to be sorted out first, but then again I dont want to be even more behind because im already 20 years old. Some of the problems that I have right now is: I have acne, I look about 16 and feel about that age as well, I have braces, I have a tooth missing (The one next to the front teeth/ I getting an implant after braces), Im shy and have low confidence when it comes to getting girls. Some good things about me is, Im very nice when you get to know me, I have a car, Im going to college and I have a job (banquet server). So my question is what should I do to get myself a girl (Sex isnt really that big of an issue with me, I just wanna have a girl to have some kinda of relationship with).

Reply:don't worry about it. if you search youll find someone horrible but the second you say your done with this someone will show it happened to me and he's perfect. just live your life and she'll come to you. so dont search all the time just explore and she will be there and when you see her smile you will know. then just go up and talk to her.
Reply:These things tend to happen when your not looking. Which is hard to do. God lets us have things when we can handle them and or to learn from. Just a suggestion not that you looked that bad or anything but that proactive seems to work really well for acne and doesnt seem that expensive. I think you just need to start getting agressive and asking girls out or for what your interested in.

a few years back, my confidence level was almost 0. i was bullied for quite a few years in school. in fact, from age 9, till i stopped school when i was 15. im disabled. i have 2 crooked legs and a crooked spine. i have treatment in hospital 3 times a week and will eventually need a kidney transplant.





what i HAVE found though is that there IS at least one friend out there for everyone. you may not find them immediately, in fact it may take you years to find them. but you WILL one day. life isnt all bad.





as for qualifications, check out open university. www.open.ac.uk correspondence courses, so you dont even need to leave your home. and theyre cheaper than normal colleges/universities.





as for physical looks? the people that matter wont worry about how you look. it took me YEARS to realise that, but it is true.

And it is okay to use the welfare system, that is why it was made to help people that are suffering. There are some drugs (ecstasy) which are rumored to be promising for PTSD, so maybe that can be solved soon, expecially if you volunteer for a trial.





You might try taking long walks in the sun. This could help physical *and* mental health.





Some philosophies have that no possessions are a good thing.





If you study, you may be able to become an expert on your problems, and help other people similarly afflicted.

Reply:I read a few years back that yoga has been successful in helping people deal with PTSD. It may also help to relieve some of your physical pain by stimulating circulation gently. Most yoga studios have special classes for people with injuries, or verbally make suggestions on how to modify postures for people who are injured. Yoga classes tend to be nonjudgmental places that are open to everybody, regardless of your background or abilities, and many facilities will let you do small chores on site (like cleaning or folding towels) in exchange for classes, if you can not pay. You could call whatever yoga facilities are near you, and ask if arrangements could be made as such. You could visit the facility in advance, meet the instructor, and see what the place is like before you attend, so it's already a familiar environment, if you do wish to join them. You can also tell them precisely what you have shared here. They will most likely understand.





I suffered from PTSD and depression for years, and it took a serious toll on my health, leaving me completely bedridden and incapable of completing even the smallest tasks within my own home. I found that starting anything was an enormous hurdle that would send my heart racing and leave me terrified to even put my feet on the floor, so that the only option was to dive back under the covers. I genuinely found that joining a gym helped a great deal, both with the physical issues that had arisen from the lifestyle changes that depression had forced upon me, along with the side effects of antidepressants, and also with the emotional blockages I faced. Getting out the door was the hardest part, but I chose a 24-hour facility, so that if 3am was the only time I felt brave enough to do it, I could.


Also, I hope that you are still seeing a professional to help you deal with all of this.I know that sometimes when a person has been involved with in-patient treatment, it is common to feel fearful and mistrustful of mental health workers after. If this is the case with you, please reconsider. Talk to a therapist, and maybe try to find a support group. Perhaps in a support group, you will meet people who understand what you're going through, and will be able to understand your challenges enough to not run away from them. You may find friendship there, and a network of people who will pool resources with you, and help you get back on top.

Once you are working you will have entered the world of possibilities as you will have money. Money solves a lot of problems. When you have money you can see a therapist, fix yourself up, and go out on dates. So really all you need is a good paying job. You can get one if you have language skills, especially at schools and universities.

Reply:wow!! well one thing about you is your a great writer! and very able to put down your feelings, i have bipolar disorder, and suffered many similar feelings to you. after going through severe traumas in my life i am now engaged to a wonderfull man, my advice to you is, tell her slowly, dont make her feel like you have 'something wrong with you' let her know, this is a small but important part of you, a part which has made you as a person better,more sensitive to others and stronger and more caring. Be honest with her, dont tell her all at once, and most of all belive that if she does 'run a mile' once she knows of your history then maybe she isnt worth being with at all!!

Reply:first of all after you've built up a friendship you need to be honest with her about what's going on in your life. Before you do that you need to not sound whiney, merely state the facts. You want to visit another country but that's costly and you don't have the money to do that (you said), and, unless marriage were in the picture I don't think you could just go to another country without being able to support yourself and being a professional at what you do. So I think you are over dreaming this and you're going to get disappointed and depressed over it.





If you have been bullied, mugged, beat up or whatever then you need to get some form of employment (there are testing places run by your county or state to see what work you can do, and then they help you find work, even piecework) and you should move to a better neighborhood, even if you have to share an apartment for a while till you can make it on your own. Having decent income and a career are two things that attract females. Learning a skill or a new skill and supporting yourself does cure the self-esteem problem. If you feel uncomfortable with your looks you can later work from home or in an office wherein you don't have to meet clients. Here in the US there is paid education and assistance to find work thru the state and government for people with disabilities, check out to see where that is located in your country. Closing Front Tooth Gap?

i damaged my front teeth agers ago, my teeth look okay on the eye, but don't feel good since the damage.





anyway i'm paying private once the new year is over with and possibly looking at 2 dental implants, or maybe 1 implant and a procera crown.





just wondering though, my teeth are pretty straight but i have a slight gap in my front tooth, could the dentist easy make the teeth bigger should i decide to get an implant (or crown) to cover the gap?





i don't want to bother with 'bonding' as you need to keep getting that done every few years, but i'm unsure whether to keep the gap or join them together.

Reply:I have two implants %26amp; it was a lengthy business having them done (nearly a year from start to finish) %26amp; not particularly pleasant (had to wear false teeth for months,had a bad reaction to the painkillers I had to take after the bone graft %26amp; insertion of the implant) so it is not something to be undertaken unnecessarily. If your teeth are in reasonable condition, a simple crown is probably all that is needed and only if you are really unhappy with your appearance. I also have a veneer %26amp; that has lasted well so far. A gap between your front teeth is meant to be sexy %26amp; slightly imperfect teeth look much more real than perfect ones. Will it make you happier to have your teeth fixed, or will you then find something else wrong? If you aren't working %26amp; having your teeth fixed won't make it any easier for you to find work, then I would suggest saving your money.

Reply:It is a lengthy and expensive process. I have bonding/veneers on my front teeth (to close a gap too) and it has lasted over 7 years and still going strong. They now also make them so you can bleach them with the rest of your teeth if you like. I didn't like the idea of completely removing a tooth for a simple gap. Implants into the bone are painful, my sister is a dental hygenist and says people only choose them if they are missing teeth or have to have a lot pulled out. Be careful

This is a strange request, I would be interested to know why you want this procedure done. Not many Dentist's would even be able to carry this surgery out, unless they are specialists and you certainly won't get it done 'cheap', it would be something that a private Dentist may do, or a Dental Hospital may do it, but only for valid clinical reasons. Yes, you will lose bone in that area as long as there is nothing occluding with it, but having an implant will actually stimulate bone regeneration. Don't worry, you will have plenty of bone for the implant.

Tooth and bone are two different thinks. when we extract a tooth of course we dont extract bone. now the empty hole will fill with bone.


BUT


1.how deep is this hole ? if the tooth was extracted becase of periodontal desease and it was more or less loose then that "hole" is not deep and may not exist at all


2. how vital is the remaning bone ? if the tooth is lost after an extensive absses then the bone is not so vital to fill the "hole":


3. your age ! I m 50 and at least my bones were stronger when i was 20.


I think this is the way you should approach you question. I think many other reasons may affect how much bone will made in the socket and nobady can tell exactly.

Reply:THE AMOUNT OF BONE TO HOLD AN IMPLANT DEPENDS ON WHERE THE IMPLANT IS TO BE PLACED.


IF THE IMPLANT IS TO BE PLACED IN THE UPPER JAW, IT SOMETIMES CANNOT BE PLACED WHERE THE UPPER SINUSES ARE INVOLVED. THE AMOUNT OF BONE PRESENT IS IRRELEVANT. THE AREA WHERE A TOOTH HAS BEEN LOST WILL EVENTUALLY FILL WITH BONE BUT BECAUSE OF ANATOMIC RESTRICTIONS AN IMPLANT MAY NOT BE POSSIBLE.
Reply:it is not really bone loss, per sey. the tooth is in a socket and there is bone around it. when it is pulled or falls out, the bone fills into that spot via osteoblasts, which are specialized cells that form bone. at least it that is the way it is supposed to work. unfortunately, non-biologic dentists do not know to pull out the periodontal ligament to allow the bone to form. that creates a cavitation. you should not get an implant. they are not a good thing.





CAVITATIONS





A cavitation is an unhealed hole in the jawbone caused by an extracted tooth [or a root canal or an injury to a tooth]. Since wisdom teeth are the most commonly extracted teeth, most cavitations are found in the wisdom tooth sites. Please see the graphic and photo below to get a glimpse of what may be in your mouth and the effects it is having. The photo and diagram demonstrate the destructive and pathologic consequence of a routine tooth extraction. Dentists are taught in dental school that once they pull a tooth, the patient's body heals the resulting hole in the jawbone. However, approximately 95% of all tooth extractions result in a pathologic defect called a cavitation. The tooth is attached to the jawbone by a periodontal ligament which is comprised of "jillions" of microscopic fibers. One end of each fiber is attached to the jawbone and the other end of the fiber is attached to the tooth root. When a tooth is extracted, the fibers break midway between the root and the bone. This leaves the socket (the area where the root was anchored in the bone) coated with periodontal ligament fibers.





There are specialized cells in the bone called osteoblasts. Osteoblasts make new bone. The word "osteoblast" means bone former. They are active during growth and maintenance. However, the periodontal ligament prevents the osteoblasts from filling in the tooth socket with bone since the periodontal ligament fibers lining the socket act as a barrier beyond which the osteoblasts cannot form bone. In other words, an osteoblast "sees" a tooth when it "sees" periodontal ligament fibers. Since there are billions of bacteria in the mouth, they easily get into the open tooth socket. Since the bone is unable to fill in the defect of the socket, the newly formed "cavitation" is now infected. Since there is no blood supply to the "cavitation" it is called "ischemic" or "avascular" (without a blood supply). This results in necrosis (tissue death). Hence we call a cavitation an unhealed, chronically infected, avascular, necrotic hole in the bone. The defect acts to an acupuncture meridian the same way a dead tooth (or root canal tooth) acts. It causes an interference field on the meridian which can impair the function and health of other tissues, organs and structures on the meridian. Significantly, the bacteria in the cavitation also produce the same deadly toxins that are produced by the bacteria in root canals (see Root Canals). These toxins are thio-ethers (most toxic organic substance known to man), thio-ethanols, and mercaptans. They have been found in the tumors in women with breast cancer.

A titanium dental implant is usually made out of an alloy of this metal along with several other metals blended together. The most common alloy used has a ratio of 90 parts titanium, 6 parts aluminum and 4 parts vanadium





titanium implants are putting metal and a high milliamperage close to the brain. i have seen a woman who measured nearly 400 milliamps positive charge, and 30 neg charge who had three in her mouth; she paid $9,000 for them. the dr said she could have ran a stereo off her teeth. we removed them. our body runs on electrical impulses, so this can disrupt them (and brain waves). also, dentists and drs will tell you that bone grows to titanium implants. well, it will grow around it. but, it is a foreign object and the body will build up antibodies to it. over time, it will pull away from the bone and can become loose. if you will notice, they say implants last about 15 years or so. they are working on an implant made of diamond, supposed to be available in 5 yrs. but, it will still be a foreign object and pull away from the bone.

Block Bone Graft of my bone from Chin, Jawbone, or Hip (High Risk) Chin This procedure requires cutting through the inside of the bottom lip (he cannot cut along my gum line) and removing the bone from your chin. I do not have enough tissue in my gum area to cover the surgical site. Tissue may be used from underneath the tongue to create the flap. It will take approx. 6 months to 1 year for the implants to integrate into my jawbone. Crown all 3 implants separately. Keep yourself strong and healthy and hope that it works for a very long time. Have frequent cleanings (every 3 months). I have decided to not use bone from my face or hip. I will attempt either cadaver or artificial bone grafting.

2.7.6. Titanium


It is generally accepted that pure titanium is extremely well tolerated by local tissues and induces neither toxic nor inflammatory reactions (Branemark et al. 1969, Toth et al. 1985, Linder et al. 1988, Pfeiffer et al. 1994). The normal tissue concentration of titanium in humans is 0.2 ppm. Around the titanium implants no clinical tissue toxicity has been observed even at local concentrations higher than 2000 ppm (Hildebrand et al. 1998). In optimal situations, titanium is able to osseointegrate with bone, thus forming a direct contact with bone at the light microscopy level (Branemark et al. 1969). The good bone contact may be due to the ability of titanium to form a Ca-P rich layer on its surface (Hanawa 1991). Titanium is bacteriostatic (Elagli et al. 1992) and does not significantly activate or inhibit different enzyme systems specific to toxic reactions, e.g. β - glucuronidase, lactate dehydrogenase, glucose-6-phosphate dehydrogenase and acid phosphatase (Elagli et al. 1995). The good biocompatibility and corrosion resistance are due to the naturally forming stable titanium oxide (TiO2) film on titanium surfaces (Zitter et al. 1987, Kasemo et al. 1991).





Particles from titanium arise from the passivation layer of the implant, but they are not titanium ions, but mostly insoluble titanium oxides or suboxides, which are recognized to be biologically inert. Indeed, the passivation layer is immediately reformed after abrasion because of the high oxidizability of titanium. This behavior protects the alloy and prevents the formation of chemical compounds other than oxides (Hildebrand et al. 1998). Tissue discoloration due to titanium oxide particles is sometimes seen around pure titanium implants, but this seems to have no clinical consequences (Onodera et al. 1993, Rosenberg et al. 1993). Experiments with laboratory animals and some limited analyses of human tissues have also revealed evidence of titanium release into distant tissues (Schliephake et al. 1993, Jorgenson et al. 1997).





Wear particles produced by abrasion appear especially in the vicinity of articular prostheses and implants with certain mobility, e.g. uncemented total hip replacements. These particles may induce multiple tissue reactions, including osteolysis, degradation of normal bone structure, severe macrophagic reactions, granuloma, fibrotic capsules and chronic inflammation, which may cause destabilization and loosening of prostheses and implants (Santavirta et al. 1991, Santavirta et al. 1993, Rubash et al. 1998). Particle size and composition are of essential importance in that process. Deleterious reactions have been reported with Ti-6Al-4V based prostheses (Nasser et al. 1990, Rubash et al. 1998), but not with pure titanium implants.





In vitro, pure titanium particles have also been shown to have some effects on cells. Low concentrations may stimulate fibroblast proliferation, while high concentrations may be toxic. At high particle concentrations, titanium caused a decrease in proteolytic and collagenolytic activity in the culture medium. Titanium also elevated the lysosomal enzyme marker, hexosaminidase, except at high concentrations (Maloney et al. 1993).

Metal-on-metal hip bearings are being implanted into younger patients. The consequence of elevated levels of potentially carcinogenic metal ions is therefore a cause for concern. We have determined the levels of cobalt (Co), chromium (Cr), titanium (Ti) and vanadium (Va) in the urine and whole blood of patients who had had metal-on-metal and metal-on-polyethylene articulations in situ for more than 30 years. We compared these with each other and with the levels for a control group of subjects.We found significantly elevated levels of whole blood Ti, Va and urinary Cr in all arthroplasty groups. The whole blood and urine levels of Co were grossly elevated, by a factor of 50 and 300 times respectively in patients with loose metal-on-metal articulations when compared with the control group. Stable metal-on-metal articulations showed much lower levels. Elevated levels of whole blood or urinary Co may be useful in identifying metal-on-metal articulations which are loose.

Zirconium dioxide implants are supposed to be the wave of the future. They are still putting a foreign body into the jaw and the immune system will launch an immune response, so they will still loosen over time (15 to 20 years) from that. Granted, it appears to be better than titanium and they are saying it is a substitute for metal implants, but with the immune response, it isn't worth it to me.

Properties





The German chemist M. H. Klaproth discovered zirconium dioxide in 1789 although this "miracle material" with its outstanding properties has only been re-discovered in the last few decades. For instance, various types of zirconium dioxide have been introduced to dentistry as a substitute for metal. This material is attractive because of its extraordinary properties such as high flexural strength (in excess of 1,000 MPa), hardness (1,200 – 1,400 Vickers) and Weibull modulus (10-12). Yttrium partially stabilises zirconium oxide to provide these positive properties. Adding aluminium oxide boosts the flexural strength of the zirconium dioxide alloy once again. Zirconium dioxide is used for manufacturing kitchen knives, industrial cutting tools and components under great thermomechanic stress in the automobile and aircraft industry. However, it is not only very strong, it is also biocompatible so that zirconium dioxide is also used in medicine (hearing devices and artificial fingers and hips) and dentistry (pins, crowns, bridges and implants). The fact that zirconium dioxide has the same colour as teeth along with its biotechnical characteristics mean it is used for manufacturing biocompatible, high-quality and aesthetic tooth and implant reconstructions. There have only been animal experiments and laboratory examinations on applying dental zirconium oxide implants to date, meaning no long-term data exists on the clinical application of these implants.





Manufacturing Zirconium Dioxide





The mineral zirconium (ZrSiO4) is the main raw material for zirconium dioxide while melting it with coke and lime (reducing the SiO2) produces ZrO2 for industrial uses. Since extremely pure constituents have to be used for producing high-performance ceramics, special ways to synthesise it have been developed for high-purity ZrO2. This includes production with reactions in molten salts, reactions in the gaseous phase, hydrothermal powder synthesis and the sol-gel process. Gaseous phase and sol-gel process production provides powder at very small particle sizes ranging from 0.01 to 0.10 µm. This powder is then mixed with additives to create what are known as green bodies with film casting, slip casting or drying pressing. We distinguish additives such as sintering additives (that have a specific effect on the sintering behaviour and the properties of finished ceramics) and auxiliary materials that facilitate shaping. While the sintering additives stay in the ceramics, all residues of the auxiliary materials (mostly slightly volatile organic compounds along with water) are removed from the moulded component before the sintering process. The green body is passed into the raw product by sintering and ground or polished depending upon use. The sintering process can be carried out at atmospheric pressure and under high pressure and it is only with the sintering process that the moulded components receive their actual properties. The ceramic powder particles are compressed by lowering the specific surface with temperature-dependant diffusion processes with alternating components of surface, particle size grading and volume diffusion. If solid body diffusion is too slow, sintering can also be carried out with a liquid phase or under pressure, the latter being called hot pressing or hot isostatic pressing (the HIP process). The velocity of solid body diffusion can be boosted with the right selection of sintering additives. A great deal of research needs to be done here since the high sintering temperatures (in excess of 1,200° C) and manufacturing under pressure causes production costs for ceramic components to shoot up. Along with providing systematic clarification of the impact that additives have on the sintering process, there are also attempts to enhance power transmission onto ceramic components by coupling in microwaves for lowering sintering temperatures.





ZrO2 Ceramics





The properties of ZrO2 ceramics substantially pivot on the chemical composition of the material and the manufacturing process. We distinguish fully stabilised ZrO2 (FSZ „fully stabilized zirconia“) and partially stabilised ZrO2 (PSZ „partially stabilized zirconia“). It can be partially stabilised by adding 3-6% CaO, MgO or Y2O3 and depending upon the conditions of manufacturing this stabilises the cubic, tetragonal or monocline modification. Partially stabilised ZrO2 demonstrates high thermal fatigue resistance, meaning it fills the bill for use as high-temperature mechanoceramics. Adding 10-15% CaO, MgO or Y2O3 also allows cubic modification of the zirconium dioxide from absolute zero to the solidus (FSZ) and the ceramic material is thermally and mechanically stable to a temperature of 2,600°. However, its low caloric conductivity and higher thermal expansion factor as compared with partially stabilised ZrO2 mean that the thermal fatigue resistance of the fully stabilised zirconium dioxide is lower. The zirconium dioxide that is suited to use as an implant has the following composition: 95% ZrO2 + 5% Y2O3.
Reply:When we do an implant in our office, it is always best to place an implant the same day of the extraction.





If an implant is done at a later time, a patient should wait 3-6 months for complete healing, to see how much bone is present. At this time, you may need a bone graft. And again wait until the dentist determines when it would be good to proceed with implant surgery.





Everyone heals differently, smokers and diabetics are slow healers. So the amount of bone and the time to re-evaluate differs from patient to patient.

Will I be able to fix my jaw?

Kinda a long story, but I had braces when i was younger and I was very stupid and didn't take care of my teeth. Needless to say, I had to have them taken off and had to get all my front teeth crowned and one implant. My question is: will they still be able to fix my bite and get me ready for orthognathic surgery? My jaw looks absolutely terrible (one side goes back farther than the other and I ahve an overbite). I know it's more complicated with crowns and an implant, but is it possible? I'm so unhappy with the way my jaw makes my face look .. I've even considered suicide (i know this isn't the section for that)... I just want re-assurance that this can be corrected.

Will I be able to fix my jaw?
Holy Cow!!! Yes, they can fix the problem. At the very most you may want your crowns redone if the alignment is not right after surgery, but I bet it will be fine with these crowns. Orthognathic surgery is very common these days. You'll be back in braces for a while afterwards, and you'll most likely lose some weight because of your jaw being wired together for a few weeks. NOTHING is worse than not being here.....surgery, crooked jaw or anything else. Hang in there and good luck.

HELP! I broke a crown over a dental implant!?

In one of my front teeth, I have a dental implant, which naturally has a crown over it. Now, being the idiot I am, I managed to crack it all the way across the back, and it broke. The whole bottom half and front piece (which I guess it connected to it) are wiggly and loose to the point where I fear if I eat or drink anything, it will fall off. I called my dentist, she's having me come in first thing tomorrow morning. In the meantime, what do I do? If it falls off, will it hurt me? I know I might cut my tongue on the edge of the top piece, but would anything else potentially harmful happen to me? The screw can't come out while I'm asleep or anything, right?

Without seeing what's going on, I cannot make a diagnosis.





Crowns, when they fail, fail catastrophically. This means that something breaks off completely. The fact that something is still attached to the implant and wiggling, however, suggests that the abutment screw is loose. The crown may certainly be damaged and in need of replacement, but my concern is the implant. Implants, just like tooth roots, can fracture (which would also cause the screw to be loose).





As for the whole thing coming off in your sleep, it's certainly possible. I would recommend sleeping with your head propped up.

maybe the implant is placed at an acute angle which is causing undue stress to the post.


if the crown is not removable then it will need to be cut off and the post will need to be resecured.

Some answers about partial dentures?

i got my partial denture today (to fill the gap of the missing teeth, until i get the implants).





the denture is very tight, which the dentist said it would be so it doesn't fall out. but there abit of gum thats being squashed and its all red and i can taste abit of blood and theres pain in the tooth next to the fake teeth (i'm guessing because of the pressure to hold the denture), will this pain stop over time? and how to ease the pain?





is it also true that i have to remove the partial denture every night? (im worried that the pain will be more next day if i keep taking it out)

Some answers about partial dentures?
A partial denture can cause as much pain and discomfort as a full denture, dentures are like new shoes I'm afraid, they wont mould to your gums straight away and take some time to get used to.


What you need to do is wear the denture as much as possible so when you go back to the dentist for an EASE the dentist will be able to see exactly the places that are causing you the most discomfort and he/she will be able to file these areas.


The after care for partial dentures is the same as full dentures, you should keep them clean by scrubbing with soap and cold water, as they are partial dentures you have a higher risk of plaque retention forming, and you should remove them every evening and leave them to soak over night.


If you fail to keep them clean and stick to a good oral hygiene routine you could end up with a painful condition called


Denture Stomatitis, this is where the soft tissues covered by the denture become sore and inflammed.

Reply:Yes, you have to remove it every night. its for the best! see how the next few days go with it, if its still hurting your gums you may have to go back to the dentist and get the denture eased, i.e get a part of it filed down a little. only takes a few minutes but you might have to go back a few times until it feels fine for you.
Reply:That is painful for most patients. At my office we rarley see anyone go with a partial that doesn't need a few adjustments. They are made to fit your mouth but they usually have a few spots that are rough, or tight on an area, they can be tight next to your other teeth that part is probably fine although they can be too tight but as long as you can get it out you are fine. I agree with your dentist on that oart. i would however visit them for a quick adjustment if your have a spot making your sore. Alos yor tissue will heal much better if you take it out at night. Way to go with the implants, you can't go wrong there. They are a little pricey but well worth it I have seen great sucess with them. Good Luck. (it could take a couple adjustments to make the partial fit comfortably)
Reply:My boyfriend recently had to get partials for 2 teeth he lost in a fight and he only made 2 dr. visits. The first to have his mouth measured and the second to pick them up, he never complained about pain and he said they fit really good. He doesnt even use any fixodent or any stuff like that cause he hates the taste and his teeth stay in good. He takes them out sometimes at night and sometimes he don't and he has no problems at all I guess it would just depend on you. His look like the real thing you can't even tell their partials he was going to get the implants but now he's thinking of just keeping the partials because he found out when you get the implants your gums turn black where you have no teeth. My friend got the implants and her gum area is black due to the dead nerves and it looks funky.
Reply:If the partial denture is pressing into your gums and causing sore spots, you need to go back to the dentist for adjustments.





Adjustments are almost always needed when a denture is delivered.. Some people are lucky and never needed adjustments, but those are more exception than the rule.





Yes, you need to take out the partial denture every night. People who don't take out their dentures at night tend to develop fungus/yeast infections and other such problems.

Reply:to be honest pain during the first few days is kind of normal because it is a new thing inside your mouth and your tongue and gum tissue should get used to it, but if the gum under it is impinged ( look white or light pink as a result of pressure) then you should go back to your dentist to relieve some part of it, you may get ulcer as well if the fit is not good, if you feel tightness because of the metallic clasps and if it doesn't go after a week or so also you need to make some changes.


about taking the partial denture out yes it is better to take it out and clean it it will give a rest to your gum teeth and bone as well as prevent any possible yeast infection as you can not clean it properly when it is inside your mouth specially underneath it.

Well, an implant is a fake tooth too. I'm going to guess you mean a bridge.


Implant: Titanium post that fuses with the jaw bone and a crown is put on top of it. Brush it, floss it, eat with it like a regular tooth. Initial costs high, but long term costs low.





Bridge: carves down teeth on either side of a space and crowns each tooth with a fake tooth suspended between.


Brush it , chew with it, needs special flossing. get 10 or so years out of it and then do it again. Risks: Carved down teeth may need root canals, or worse (I've seen this alot), carved down teeth so damaged that they need to be pulled in the future, so now what? Initial costs less than an implant, but longterm costs higher due to replacements and possible root canals.

I'm not a dentist, however, an implant is actually attached permanently into the jawbone and probably costs around $2500. By fake, I guess you mean a crown...the dentist drills away part of your original tooth, leaving a "peg" to glue a crown or "fake" tooth to. That usually costs between 6-8 hundred, depending on where you live.
Reply:Technically, an implant IS a fake tooth. An implant is a porcelain crown glued to a titanium screw that has been surgically implanted in the bone. A bridge has a pontic (the fake tooth) and abutments (the crowns on the remaining teeth on either side of the missing tooth). A bridge is all one unit, everything hooked together, so you cannot floss in between it. You have to use a floss threader to go underneath it. An implant is like a separate tooth that you can floss. Dentures at 46???

I'm only 46 years old, and two specialists have confirmed that all my teeth need to be removed. They are deeply decayed, and many are abcessing. This is because of a medication which causes dry mouth. I've poured a fortune into my teeth already.





I need advice. I'm in pain and scared to death. I don't want to wear old-people dentures, and I don't want to talk like an old person.





Has anyone young been through this? My self-esteem is hitting an all-time low, thinking of my reflection in the mirror with no teeth.





We can't afford implants all over. The best we can do ($30,000) is overdentures.

look there are children born with gum dieases and will eventually have to have dentures by the time they are in their 20's. i know people who have had dentures since they were in their early 20's. you will be the only one who will know that you have them. you can get them custom made however you want them to look like. you can get them made to wear a front tooth has a little chip in them. "very natural looking". you won't look like some old person. dentures are not for just old people. trust me your self-esteem level will go through the roof when you get your pretty white dentures. you'll look sooooo much younger compared to the way that you look now. you won't be disappointed. would you rather wear dentures or walk around with no teeth or black rotten teeth. at least you care enough about yourself to where you want to look your best. i see alot of young people walking around with no teeth and black teeth. you need to give yourself some credit and get the beautiful smile that you deserve. good luck.
Reply:A good dentist should be able to make you a good set. Look into finding a dentist that does mini implants. These are smaller implants put in the bone to hold the dentures down. These are much cheaper than full mouth implants but this way you don't have to worry about your denture slipping around and you looking like an old person. Lower dentures are always hard to keep from slipping without mini implants and the mini implants will help you hold onto more of your jaw bone after the teeth are extracted so your dentures will fit better also. Mini implants for dentures is a specific type of implant that is small and cheaper than the implants bars or full size implants used for dentures. I would expect this whole process for implants and dentures top and bottom to be anywhere from $4,000-$6,000 depending on area. I do not know how many teeth you have to extract. They place the implants when they extract the teeth. Normally, you only need 3 mini implants top and 3 bottom.
Reply:You know what, don't worry. You know that you did the best you could to take care of your teeth and that is what matters. I know that it will probably be tough to adjust at first, but just think that at least you won't have to be in anymore pain from abcesses and stuff after this. Don't be scared, I am sure everything will be okay. Also if you get a good set, it is likely other people won't even notice. Maybe you could get implants over time...save up or something. Everything will be okay. What matters is that you are doing the best thing for your health right now. Hugs and hang in there! Everything will work out just fine. :-)





p.s. My mom is only slightly older than you and has had dentures since she was in her early 40's! Nobody can even tell, and she doesn't look old at all, nor does she talk like an older person. We forget that she has them unless she mentions them or something. She puts on her biggest smile and most of the time doesn't even think about dentures. :)
Reply:You know you have to eat and if dentures is the way to go then do it. I was 23 when I got my dentures and I don't talk funny or like an old person. If you want to keep your good health you will get the new teeth. And don't be ashamed of yourself for doing this for yourself. They make teeth look just like the teeth you had before they rotted out. I'm 55 yrs now and most people don't even know I have them unless I say something. they will put the new teeth right in after they get them. And reline them as needed.
Reply:Yeah - I think mini implants would be the way to go for you if you can afford them. They're a lot cheaper than individual implants and a lot less trouble than removable dentures. Try to think positive - once you've had it done, you wont have all the pain of abscesses and decay any more and with the implants, you wont be losing bone so you wont get that caved in look which results from bone loss. Good luck - I'm sure you'll be OK.
Reply:Have had to do dentures on a 16 yr old but because of gum desease, not by choice!! Hey good fitting dentures can look great %26amp; many function well but never like your natural teeth. Just be glad if you can afford them %26amp; make the best of a not so great situation. Have made many for patient less than 46,it ain't the end of the world, spose you had cancer!! DMD here

She wears dentures and they did a really good job on them... if you didnt know , you wouldnt notice. Proper fit is important.
